BASIC DESCRIPTION
Home health care nurse practitioners provide home-based care to chronically ill patients across age groups. They conduct assessment, diagnose disease conditions, order and provide treatment, and evaluate patient care. They also provide referral services if necessary. They work with members of the health care and maintain close communication with patients and caregivers.
EDUCATIONAL REQUIREMENTS
Registered nurse preparation, a master’s degree and Nurse Practitioner certification are required.
